Unable to Remove Floating Contact form

Hi,

I am using Aegaeus theme. I have a page set up that was originally set as a contact page using the contact page template. I decided to switch to the default page template and insert a ninja form. However, the original form that was on the contact template is still showing up on my page – it’s just hovering over the new content I’ve added in.

I’ve cleared the cache and have waited some time and been refreshing the page but it still shows up.

Can you help me fix this, please?
