Post seems to disappear when I hit "publish"

Apologize in advance for my lack of tech knowledge. Here is what has happened about 4 times out of approx. 120 posts: everything looks good. I hit “publish.” And then when I go to my email notification to double check everything, I click on the link to the post and it takes me to essentially a blank page on my site.

As an example, the top of the post is showing: Home > Meatloaf > Meatloaf

What it SHOULD be showing is: Home > Recipes > Mains > Meatloaf

It can’t be operator error because, as I’ve mentioned, I’ve posted 120 times and this has occurred only 4 times.

Anyone know what is happening and how I can correct it when I notice it? I’ve got 60 “subscribers” so every time there’s a mistake I’m blasting it to 60 people. Embarrassing!

So far, I just re-do the post, cutting and pasting from the messed up post to the new post and then everything is fine. I’m baffled and frustrated! Thanks!
