Lost Code Contact Form

I deleted the code from my Highend Contact form. When I copy and paste it back my contact form does not work; it just shows the submit button and Capthca. When I submit the form and use the Captcha this is what I get:

From: [your-name] <[your-email]>
Subject: [your-subject]

Message Body:
[your-message]

The form does not appear on my contact page.
