lets get in contact page

Hi I am using one of the HB themes and the “lets get in contact page” is contact form 7 – I actually need to do the following – I want to have a form that I can put a Name, phone number, email address, company name – then I want to list 6 ?? questions to qualify the person filling out the form – they will need to type in answers to – what is the best thing to do and how do I get it to interact with the HB Theme?. Do I have to download Gravity Forms or something similar kind regards Andrew
