Contact Form – Highend Set up

Hello

Are there some instructions for setting up the contact form? I have installed the contact form 7 plugin, but I am not sure which fields to fill in with my details.

I have attached a screenshot of the fields I think I should be completing, but when I did, I did not receive the message I sent to myself to test the form.
