Problems Adding Mailchimp form & 3rd party widgets

I can’t seem to add in any external widget/plugin to this theme. Is there another way to include a Mailchimp form, using the Mailchimp plugin, where I added the shortcode to the page, http://www.myconciergedesk.co/signup/ and notice there is a snippet of code appearing [contact-form][contact-field label=’First Name’ type=’text’/][/contact-form] right above the form itself.

The reason I want to ask about the theme specifically, is I also created a “coming soon page” using launchrock.com, where I place created JS within the page to reveal the created widget, but I get this http://www.myconciergedesk.co/productpagelaunch/ “can’t load site, Bad!” So my question is are there limitations or restrictions in adding either Mailchimp forms (again, I’e installed the plugin, and followed all mailchimp protocol) or other 3rd party widgets to any of the theme pages?
