No Image Widget, HTML In Text Widget Doesn't Work Either

A few months back my image widget disappeared somehow. I have gotten around it by typing html into the text widget instead. But now, this is not working either. I tried to add an image with a link to a page on my site. It worked in the preview, but disappeared as soon as I published it. I have redone it 3 times, double checked my code/picture/site, saved everything, and it definitely isn’t showing up where the widget says it should be. You can see my code in the attached screen shot.
