Modal window stopped working

Hi-
I have a modal window configured on the following URL to start when the page loads http://www.patriciamnewman.com/teacher-guides/. For some mysterious reason, it stopped working. I’ve attached a screen shot of how the modal window is configured. Any suggestions for how to get it to work again?

I’m running WP 4.9.2 and my Highend theme is up to date. I cannot see the modal window on any of my browsers (Safari, Microsoft Edge, or Chrome).

Again, this window used to work. I’d appreciate your advice.
