Embedding Mailerlite Form

Can you please tell me how to embed my Mailerlite form? I have already added necessary code to the HEAD tag. I just need to know how to get the form to appear using this code:

I’ve tried using the Text Block and adding it to the “Text” tab but it doesn’t display anything.
