Add Subscription form to coming soon page

I’d like to add a way for visitors to submit their email on my coming soon page. I saw a topic on this that recommended adding MC4WP Mailchimp for Word Press which I have done and have started setting up. But I see no way to actually add it to the page. In the Highland Options -> Coming Soon Mode settings page I don’t see anything to do it. I thought maybe the shortcode generator but it isn’t there either. How can I add it to the page?
