woocommerce shopping cart product options

I’m hoping someone can help me set this up, or give me advice on how to go about it. I understand this is probably not a theme issue, more of a woocommerce question. If you know of a plugin or have any advice, it is greatly appreciated.

This needs to go on the product pages as ordering options:

Design Options
[check box or radio button] I Will Upload My Own Design – FREE
[check box or radio button] I Need You to Design My Banner – $39

These are options for shipping at checkout:
Rush Options
[check box or radio button] Standard Delivery – (3-4 Business Days) – NO Additional Charge
[check box or radio button] $15 Next Business Day Pickup – (Order by 3 PM Today for Pick Up Next Business Day)
$24 Same Business Day Pickup – (Order by 12 Noon for Pick Up)
