Contact forms

Hi guys
The Contact FORM on the contact PAGE is from the option to build a new page using the contact page template . I like the contact page template because of the layout and MAP option at the top. BUT every time I alter the contact 7 form options they do not appear in the saved form.

So my questions are.

1- can I change the actual contact page template to use another (custom ) form? (my form)
2- or how can I make a new Contact ME Page look like your template with its layout and map?
