Contact form unusable – HELP

Hello,

OK. I set up a contact form and it won’t allow ANY other features on the page, even if I embed the short code inside of a column, it won’t allow any other content on my page.

I want to know how to set up a contact form that looks and works like this:

But mine looks like this no matter what I do (there is lots of other text on the page before the contact form but you can’t see it), even if I start with a new form:
