Upload Avatar in Profile Builder plugin not working

We are using the Profile Builder Plugin (https://nl.wordpress.org/plugins/profile-builder/developers/) to add a custom avatar to the user data. This worked fine for a while, but for unknown reason it stopped working. We’ve contacted the plugin developer but they state (correctly) the upload-button is still working with the standard WordPress themes. In Profile Builder you make an (non-standard) user-list and single-user-page and you can make an user-edit-page. I’ve tried different configurations of these pages, all combined with the correct shotcodes, but it’s not working: the button that is automatically created if you add the Avatar-field to the edit-page is not working. Normally (if you go the the back-end profile page) you press the button and a pop-up shows where you can select a file. In this situatation nothing is showing nor loading.

Can you please help us? Or at least give us some things to try? We are out of options here…
