Ninja Forms not working with Aegaeus Theme

We are using ninja forms to build a contact form, however when using the plugin it does not display the field boxes properly, Ninja Forms said it is because the theme is poorly written and has j query errors. Can you please help me fix the theme…

Here is what Ninja had me look at using the chrome inspect element console:
